OHIO SOUTH STATE LEAGUE 2012-13 Season

Team Application Instructions

Application/Payment Due July 13, 2012

Create an account

All teams applying online for the 2012-13 OSSL season will use their existing account with Affinity Sports or create a new account. If the person applying has done licensing or risk management through OSYSA, they will have an account. For forgotten usernames or passwords, click on ‘Forgot Password’ on the application home page.

Applications must contain both the team manager and head coach as contacts.

To begin the application process, follow the steps below.

A. Go to the Ohio South website: www.osysa.com

B. Click “Tournaments” and “Ohio South State League”

C. Click on the link to apply to the OSSL

D. Log into Existing Account/ Find Forgotten Account Information /Create an Account

a. Log into Existing Account

i. Enter username and password

ii. Click “New Application” tab

b. Find Forgotten Account Information

i. Click “Forget Password” and follow instructions

c. Create an Account

i. Click “Apply to Tournament”

ii. Enter contact information

iii. For “Work Phone”, enter your cell phone number

iv. For “Username” and “Password”, enter a new username and password

E. Once you have logged in or created a new account, you will be directed through the application process. Follow the steps below.

Submit an Application

A. Team Information page

Enter the following information:

a. Soccer Affiliation: choose “USYSA” or “US Youth Soccer”

b. State (Association) Name: choose “Ohio South”

c. Team Name: enter the team’s name for the 2012-13 season

d. Home Jersey Colors: enter dominant jersey color

e. What is your relationship to the team: choose “Team Manager” or “Head Coach”

f. Team Gender: click on “boys” or “girls”

g. Birthdate of Oldest Player: this response will populate the Age Group information

i. If you do not know the oldest player’s birthdate, enter 07/31/XX (choose year)

h. Please list the Head Coach First and Last name: enter information

i. Please list the Head Coach License Level: enter highest coaching license obtained

j. Click “Save & Continue”

B. Contact Information page

Enter the following information:

a. Enter contact information for head coach or manager (different from person who logged in)

b. Username and Password: you may but do not need to create a username and password for this second contact.

c. Click “Save & Continue”

C. Team Results and Record Information page

Team results have to be entered ONLY by teams who finished BELOW eighth place in their age group in this season’s Ohio South State Cup. For the list of the eight top teams per age group, refer to the OSSL website.

a. For teams that finished in the top eight, do NOT enter team result information. Click “Continue”

b. For teams that finished in ninth place and below, enter the following information by clicking on the word “Edit” in each section.

i. 2012 State Cup results (2011 State Cup results for the U12 age group)

ii. 2011-12 Primary League results

iii. 2011-12 Secondary League results if applicable

iv. 2011-12 Tournament results for two tournaments

v. Click “Continue”

D. Comments page

a. Enter any additional comments in the “Add Comments Window” by clicking in the window and typing. When finished, click “Add Comments”

b. Click “Continue”

E. Review Application page

a. Review your application and make any changes necessary by clicking on “Edit”.

b. Click “Submit Application”

F. Electronic Legal Agreement page

a. Review the OSSL Participation Agreement and OSSL Rules (which are on the OSSL website)

b. Enter your initials, first name and last name

c. Click “I Agree”

G. Submit Payment page

The league fee for the 2012-13 OSSL season is $500. Teams that are not accepted in the OSSL will be refunded their league fee. Please note there is NO convenience fee.

a. Payment Method: choose check or credit card and click “continue>>”

b. Enter the requested information and click “continue>>”

c. Review and click “Submit Payment”

i. If paying by check, click on “View Payment Summary Coupon”

ii. Print the Payment Summary Coupon and include it with your check.

iii. Checks are due in the office by July 13, 2012.

d. You will receive an email confirmation that your application was received.
